CLIMBER REGISTRATION & AGREEMENT

I AGREE TO THE FOLLOWING

Climb for a cause (referred to herein as CFAC) shall not be responsible or legally liable for any losses of property or personal injury suffered in connection with any activities associated with a CFAC event. I hereby waive all claims against CFAC, its sponsors, and any personnel from any and all liability arising out of any injury to persons or property and any damage or expense arising out of my participation in or any activity associated with this Event.

I understand that climbing is a physically challenging sport with a risk of accident or injury. I am in proper condition to participate in this Event.

I hereby give my consent to CFAC to use photographs taken during the event. Should it be required during a climb, I authorize medical treatment by a qualified medical professional.

Please accept this $1000 non-refundable payment to reserve my spot on the 2008 team of Climb for a Cause. I understand that my participation includes an agreement to raise a minimum of $2,500 if this is my first Climb For A Cause, $2,000 if I am a CFAC Alumnus, and $1,500 if I am a student.

I further understand that I am financially responsible for my personal expenses associated with getting to and from the trailhead, and for all accommodations preceding and following the Event(s). I am also responsible for any fees associated with retaining the services of a qualified professional Guide Service.

Once my application and payment are received, I will receive a call to schedule a consultation with CFAC Executive Director Daniel A. Bobrow to help me prepare for this Event.
